Your First Visit
When you first arrive for your appointment, our team will give you the individualized attention that you deserve. Dr. Jimish or Dr. Niraja will then perform a comprehensive oral examination, identify potential problems, and then recommend treatment options, if necessary, that can resolve the issue and get your smile back to full health and radiance. If you’d like, we can also provide a professional dental cleaning during this visit to remove accumulated plaque and polish your teeth’s surfaces. We hope that your experience will be positive and lead to many more successful visits in the years to come!
New Patient Forms
If you’d like to avoid spending extra time in our waiting room, you should consider completing your new patient forms before you arrive! Just click below to download, print them out, and complete them. Then, simply bring the filled-out forms to your first appointment, and hand them to our front desk staff. That way, we can check you in more quickly and bring you back to start your appointment sooner.
